ANSWER: 35 year old woman with sudden onset RUQ pain and falling hemoglobin.
1. What is the most likely diagnosis?
Ruptured hepatic adenoma

2. What is the next step?
Interventional radiology or hepatobiliary surgical referral depending on patient stability.

Our patient underwent emergency partial hepatectomy due to her unstable condition; pathology confirmed hepatic adenoma, previously undiagnosed.

The answer to last weeks case was paraganglioma. Note the characteristic location above the carotid bifurcation with splaying of the ECA and ICA. Patient has succinate dehydrogenase subunit D (SDHD) mutation which is seen in hereditary paraganglioma-pheochromocytoma syndrome. Patient had bilateral recurrent paragangliomas. Note the T2 hyperintense paraganglioma on corresponding MRI. Thanks for playing!
